Vital Records Request

The City of Salida will no longer be issuing Vital Records requests. Vital Records office will be migrating to Chaffee County Public Health. 

For certified copies of Colorado birth and death certificates please visit https://bit.ly/ChaffeeVRappointments  to make an appointment with the Chaffee County Public Health Vital Records Team. Follow the directions on the scheduler and select a time to pick up your record. We will not be accepting walk-ins at this time. 

Requestors will need current, valid identification and proof of their relationship to the person’s record being requested. Details of acceptable identification and relationship proofs are on the application forms.

Customers who wish to order certificates online may use the secure email portal and send requests to . Do not email personal information without using the secure state email portal. Customers who need to make corrections to their record, or report lost or stolen certificates should visit the Colorado Department of Public Health and Environment Vital Records page.
