Patients and guardians of minors can now use the Colorado Immunization Information System (CIIS) Public Portal to access their Colorado Immunization Record. Visit the Public Portal. Chaffee County Health Assessment & Improvement Plan 2022-2026
The 2022-2026 Chaffee County Community Health Improvement Plan (CHIP) has been finalized. (See the Press Release here.) The CHIP is developed to identify major health concerns, describe community strengths and assets, and identify community challenges and areas of improvement in Chaffee County. Additionally, the goals of this project are to engage community members in local public health decision-making as well as to encourage a diversity of perspectives from across Chaffee County.
